Staff New Hire Tech Access Request Form

The form is to be used when there is a new person hired and needs access to CalArts technologies. These technologies include user accounts, email, access to software applications, network share, phone, etc. This form is for newly hired staff positions.

As a result of submission, a CAIT ticket will automatically be created. Please allow 3-5 business days for processing. You will receive an email from cait@calarts.edu once updated.

Faculty New Hire Tech Access Request Form

The form is to be used when a constituent needs access to CalArts technologies. These technologies include user accounts, email, access to software applications, network share, phone, etc. This form is for newly hired faculty positions.

As a result of submission, a CAIT ticket will automatically be created. Please allow 3-5 business days for processing. You will receive an email from cait@calarts.edu once updated.

Equipment/Software Installation Form

This form is to be used when a constituent needs equipment or software installed on their CalArts' issued device. These could include requests for computers, laptops and other peripherals as well as software application installations. Please complete the sections below. As a result of submission, a CAIT ticket will automatically be created. Please allow appropriate time for ordering of equipment which could be delayed due to vendor shipping restraints.  You will receive an email from cait@calarts.edu once updated.

Equipment/Software Policy

All CalArts employees receive standard equipment and software in order to complete their daily tasks. If a new hire is replacing another person, the new hire will receive the old employee's equipment and/or software.

Staff Tech Access Change/Remove Request Form

The form is to be used when a constituent's access needs to change or be removed. The reason for the change could be because of moving to a new department or a change in responsibilities due to a new role or position. Removals would be for those who are leaving the institute.
