1095-C statements, beginning with 2017, are now available electronically through Self-Service.
Electronic delivery of your 1095-C is an opt-out option, meaning that if you prefer to have a paper form delivered through the mail you must explicitly make that change.
This quick guide will show you all you need to know about your 1095-C!
Login to the Self-Service
Head to https://selfservice.calarts.edu/student and login using your CalArts Credentials.
This is the same username and password you would use to login to the Hub. If you need to reset your CalArts Credentials, check out How Do I: Reset My CalArts Credentials Password?
Please be aware that the correct address is https://selfservice.calarts.edu/student, even if you are faculty or staff.
If you attempt to access selfservice.calarts.edu without /student you will receive an error in your browser. This is a requirement of the system, not a decision made by either Human Resources or CAIT.
What you will see in Self Service will be different depending on if you are a student, or faculty/staff. Please see the appropriate section below for steps that relate to you.
Faculty and Staff
When you have signed in, click Employee to proceed.
On the Employee page, click "Tax Information" to change your consent for e-delivery of tax information or view your tax information.
On the Tax Information page you will see the tab named 1095-C Information. Clicking on that tab will allow you to access your 1095-C statement.
To access it click on the the statement for the year you will like to view under Notation.
In the screenshot below, you would click on the words 2017 1095-C Statement.
As mentioned above in this article, the default option is to be opted-in for electronic 1095-C delivery. If you prefer a paper copy you will need to explicitly withhold your consent through this form.
Student
When you have signed in you will see two sections, Student Finance and Tax Information.
Since you're looking for your 1095-C, click on Tax Information.
On the Tax Information page you will see the tab named 1095-C Information. Clicking on that tab will allow you to access your 1095-C statement.
To access it click on the the statement for the year you will like to view under Notation.
In the screenshot below, you would click on the words 2017 1095-C Statement.

Changing Your Consent
The 1095-C Information tab will also be where you can change your consent status.
You can change your consent at any time by clicking on the Change Preferences button.
Once on the Change Preferences page select Withhold My Consent
After you have made your selection click Save and you are all set!
